Once you complete the new client request form below, one of our staff will get back with you within 2-4 business days to discuss your unique needs more in depth and to schedule an intake appointment.

After you speak with one of our staff, you will schedule a meeting where you will go over your needs and create a plan for your health goals.

After we create your individualized plan at your intake appointment, our staff will connect you to resources and will begin monitoring and adjusting your plan as needed.

Once the goals of your plan are achieved, a transitional plan will be created to outline next steps and how to reconnect with us if you need.
